    <![CDATA[<strong>发表人:</strong> <a href="http://www.adhd.org.cn/forum/member_profile.asp?PF=30">快乐兵兵</a><br /><strong>标题:</strong> 推拿治疗儿童注意力缺陷多动症<br /><strong>发表:&nbsp;</strong> 2016/5/28  12:43pm<br /><br /><div><br></div><div><br></div><div><br></div><div>推拿治疗儿童注意力缺陷多动症</div><div><br></div><div>&nbsp;</div><div><br></div><div>目的</div><div><br></div><div>探讨儿童注意力缺陷多动症的按摩手法治疗方法及机理。方法 单纯采用按摩手法治疗儿童注意力缺陷多动症患儿33例。常规手法包括头部手法、胸腹部手法及背部手法。根据患儿临床特征的不同，进行手法的辩证加减。结论 按摩治疗儿童注意力缺陷多动症，具有效果满意，且安全、无副作用。</div><div><br></div><div>&nbsp;</div><div><br></div><div>【关键词】 &nbsp;儿童注意力缺陷多动症 &nbsp;推拿疗法</div><div><br></div><div>&nbsp; &nbsp; 儿童注意力缺陷多动症是儿童期，特别是学龄期儿童的较常见的心理行为异常。本病是由多种生物因素、心理因素及家庭、社会问题等多种原因单独的或协同造成的临床综合征。本病缺乏特效的治疗手段，主要强调心理治疗、教育治疗、环境治疗、行为治疗和药物相结合的治疗方法，疗效尚不确切。</div><div><br></div><div>&nbsp;</div><div><br></div><div>诊断标准 &nbsp;</div><div><br></div><div>&nbsp;</div><div><br></div><div>&nbsp; &nbsp; 根据目前应用较为广泛的美国DSM-IV症状诊断标准，表现的主要特性有以下几点。活动过度：数患儿开始于幼儿期，入学后为明显。表现为烦躁不安，过多的奔跑、翻滚或爬上爬下、或与人打闹；在课堂上不时翻弄课本等学习用品，或喜欢撕纸，玩弄物品；在家时喜欢高声大叫，做作业边做边玩。行为冲动，自控能力差：喜欢打断别人的讲话或干扰他人的工作和学习，上课时无缘无故发出喊叫，与同学争吵或打架，集体活动或排队等候时极易不耐烦。注意力不集中，漫不经心：易健忘，干事容易丢三拉四。由于疏忽，经常出差错。学习成绩较差或不理想。</div><div><br></div><div>&nbsp;</div><div><br></div><div>辩证分型 &nbsp;</div><div><br></div><div>&nbsp;</div><div><br></div><div>&nbsp; &nbsp; 一般可辩证分型为心肾不足型和肝阳偏亢型。两者表现均符合上述临床特征，区别主要在面色、舌苔、脉象的判断上。前者多面色少华，舌淡苔薄白，脉细；后者面红，舌红苔薄黄，脉弦或数。</div><div><br></div><div>&nbsp;</div><div><br></div><div>推拿方法 &nbsp;</div><div><br></div><div>&nbsp;</div><div><br></div><div>&nbsp; &nbsp; 以宁心开窍益智，平肝潜阳安神为治疗原则，在治疗手法上采用推法、拿法、运法、揉法、按法、摩法、颤法及拍法等。操作步骤包括头部手法、胸腹部手法、背部手法等。①头部手法：患儿仰卧位，术者坐于其头侧。以拇指点按百会及四神聪各30秒；开天门，即两拇指自下而上从印堂—前庭交替直推50次；推坎宫，即双手拇指自眉头向眉梢分推50次；运太阳50次；拇指按揉睛明、鱼腰、丝竹空，迎香各30秒；指按人中、承浆各30秒；头部啄法1 min；头部擞法1 min；五指拿头顶30次。②胸腹部手法：患儿体位同上。术者掌推任脉，即以掌或掌根自膻中直推至曲骨穴30次；双手掌自上而下分推胸胁部30次。③背部手法：患儿俯卧位，术者立于一侧。以手掌或掌根自上而下，即从大椎直推至长强（推督脉）30次；施滚法于背腰部5 min；双手重叠自大椎至长强按压，并结合震颤法10次；捏脊10次；拇指点按双侧心俞、肝俞、脾俞、肾俞各30秒；双手掌分推背部，自上而下10次。患儿取坐位，术者立于其后。双手提拿双侧肩井5次；虚掌拍打大椎穴10次；拿双侧曲池，合谷各30次。④手法加减：心血不足型加摩腹5 min，按揉双侧足三里1 min。肝阳偏亢型加搓揉两胁肋30次。</div><div><br></div><div>&nbsp;</div><div><br></div><div>&nbsp;</div><div><br></div><div>推拿疗法如何治疗多动</div><div><br></div><div>&nbsp; &nbsp;根据对多动症辨证分型之不同，可采用不同的推拿部位。</div><div><br></div><div>&nbsp;</div><div><br></div><div>取小指末节罗纹面、食指末节罗纹面，医者以拇指分别由指根向指尖方向，直推小指罗纹面，由指尖向指根方向直推食指罗纹面。反复100～500次，通过补肾经，清肝经，达到滋肾阴，潜肝阳之功。适用于肾阴不足、肝阳偏旺的多动症。</div><div><br></div><div>取拇指末节罗纹面、中指末节罗纹面，医者以拇指向掌根方向直推拇指末节罗纹面，旋推中指末节罗纹面。适用于心脾气虚的多动症。</div><div><br></div><div>取手掌面，以掌心为圆心，以圆心至中指根横纹的2/3处为半径作圆周。然后医者以拇指沿上面的部位顺时针方向作弧形或环形推动。适用于湿热内蕴、痰火内扰之多动症。</div><div><br></div><div>&nbsp;</div><div><br></div><div>&nbsp; 结 果</div><div><br></div><div>&nbsp;</div><div><br></div><div>疗效判断</div><div><br></div><div>&nbsp; &nbsp;痊愈：各项症状全部消失，随访半年病情无复发。显效：各项症状基本消失，随访半年情况稳定。好转：各项症状明显改善，随访半年病情无明显发展。无效：各项症状无明显变化，病情无改善或有所加重。</div><div><br></div><div>&nbsp;</div><div><br></div><div>讨 论</div><div><br></div><div>, &nbsp;ADHD是无明确器质性病变的综合征，目前医学界对其发病原因、病理机制尚不完全清楚，甚至没有统一的诊断标准，更缺乏统一的客观诊断指标，现在采用的是美国精神病学会提出的诊断新标准。根据患儿的临床表现，依中医理论多属于心肾不足，气虚血亏，血不养心；肾阴不足，水不涵木，肝阳偏亢。由此导致局部或全身阴阳气血的平衡失调，脏腑功能失常，其本质是阴虚阳亢。</div><div><br></div><div>采用推拿手法治疗ADHD的指导原则是宁心益智，平肝安神。开天门，推坎宫等头部手法能开窍醒脑，镇静安神以宁心益智；推督脉，理任脉及捏脊等胸背部手法为交通、平衡阴阳之法，能够平衡阴阳，调节周围神经，促进脏腑机能和新陈代谢；腹部手法及按揉足三里能补气益血以安神；搓胁肋、拿曲池、合谷以平肝潜阳。在治疗过程中发现，患儿在治疗开始数分钟后均能安静下来，绝大部分患儿能安静入睡。笔者认为按摩治疗ADHD无副作用，无痛苦，患儿易接受。同时推拿手法作为一种接触疗法，通过手法作用于患儿肌肤，对其心理治疗也有良性作用。</div><div><br></div><div>&nbsp;&nbsp;</div><div><br></div><div>&nbsp;</div><div>&nbsp;</div><div><br></div><div>&nbsp;</div><div><br></div><div>&nbsp;</div><div>&nbsp;</div><div><br></div>]]>
